Lecture:  Improvement by Design

Assigned Reading

  1. Designing  process improvement in electronic health records.   This reading describes how to abstract business processes into database structures.  Read► Example► Slides► Listen►  You Tube►
  2. American Academy of Pediatrics: Task Force on Medical Informatics. Special requirements for electronic medical record systems in pediatrics. Pediatrics. 2001 Aug;108(2):513-5.  This paper shows that an Electronic Health Record must fit the specific details of the practice.  A generic design may do more harm than good.  Read Slides Silent video

What do you know?

Advanced learners like you, often need different ways of understanding a topic. Reading is just one way of understanding. Another way is through writing about what you have read.  The following questions get you to think more about the concepts taught in this session.

  1. A use case shows how information systems are used by different actors within an organization.  Give an example of two use cases from the reading on designing an information system for primary care providers of an HMO.  In each case, describe the decisions that are being made by the primary care provider and the information needed for these decisions.
  2. Describe how design of an information system and process change are planned together.  Give an example from the reading where this was done and list the steps that were followed.  In particular, walk through the most common reason for a visit and show how the information needed was different in the new arrangement.

"Do One" Assignment: Design How Genetic Information Can be Used in Management of Depressed Patients

Analyze the information that a clinician needs to manage a depressed patient based on their genetic information.  Identify the information needed on first call to a clinic, first visit, between first and second visit in 8 weeks, and at your recommended time for subsequent visits.   Clearly identify the point in time when you recommend that the patient receive an antidepressant prescription. Keep in mind the time it takes to have genetic information become available.  Describe who will receive the information and how this information may lead to change in prescribed antidepressant.    Present your work in a set of slides (see examples in assigned reading) with each slide describing the information needed at the specific meeting and decisions made about patient care.    End with a summary of the optimal business process your information needs assessment suggests.  Do your suggested steps reduce or increase the number of visits to the clinician?
